Clinical Review of Surgical Geriatric Patients over 65 Years of Age

Abstract
Nowadays aged people have many chances of operations irrespective of their different anatomical and physiological status. Old age has not become a contraindication to surgery anymore, so we could reduce the surgical risks by this paper, we had
reviewed
the medical records of 410 patients over 65 years of age who had received operation at the Dept. of Surgery of the Hallym University College of Medicine Han Gang Sacred Heart Hospital from the beginning of 1982 to the end of 1989.
@ES The result was as follows :
@EN 1) hospitalization and operation rate was increased comparing with the past by the increment of 6.5%, 66.3% respectively.
2) The sex ratio of male to female was 1 : 0.9 although the male to female ratio in Korea for total populations over 65 years of the age was 1 : 1.3
3) According to the disease analysis, upper G-1 tract disease was 48.9%, and benign hepatobiliary disease was estimated nearly 20% of them and was the most common one. Lower G-I tract disease was 39.8%, of which the most common disease was acute
appendicitis estimated 18.2% in frequency.
4) Tumor group occupied 28% of total diseases. The most common tumor group was stomach cancer estimate 10% of the total diseases and 48% of the tumor groups. In non-tumor group, hepatobiliary tract disease was the most common one which occurred
in
20%
of total disease group
5) The most common postoperative complication was pulmonary complication which affected 45 patients(23.9% of total disease group and 50.0% among the complication cases)
6) Coexistent diseases that might affected the occurrence of postoperative complication were hypertensive cardiovascular disease of 39 cases(41.1%), pulmonary tuberculosis of 16 cases(16.8%) peptic ulcer, 15 cases(15.8%) in order.
7) Postoperative mortality rate was 2.4% in this report.
8) Operation types were classified as elective op(282 cases) emergency op(128 cases), and those mortality rates were 1.0%, 5.5% respectively.
9) Duration of anesthesia was less than 2 hours in most cases, but the patients who received anesthesia for more than 2 hours showed 3.97% of mortality rate which was 3 times more than not.
10) The most common cause of postoperative death was sepsis and respiratory failure as a rate of 50%, 40% respectively.
